NDIS Self & Plan Managed
Assistance for NDIS Self & Plan Managed Clients - Unregistered Provider
Our services are tailored to meet the needs of individuals managing their own NDIS plans or those under plan management. We are unable to cater to NDIS registered participants operating as an unregistered NDIS provider.
Prior to commencing our services, we kindly request all NDIS clients to complete a service agreement and share their NDIS plan. This ensures that our efforts align closely with your child's specific NDIS goals.
All NDIS participants eligible for our services are charged according to the NDIS billing guide. -

Chronic Disease Management Plan (formerly known as Enhanced Primary Care (EPC)Your child may qualify for help through Medicare for Complex and Ongoing Health Needs. Clients have complex care needs if they need ongoing care from a multidisciplinary team consisting of their GP and at least two other health care providers, one of which may be a speech pathologist.
Please talk to your GP to find out if you are eligible.
How it works: Once you have discussed your eligibility with your GP, they will create a plan and refer your child to a speech pathologist using a specific form. Remember, your child won't be able to claim Medicare rebates until the speech pathologist receives this referral form.
What’s included: Your GP will decide the number of sessions, up to 5 per calendar year, which can be split among different health professionals.
Rebate details: As of July 2019, the rebate is $53.80. You can find updates on rebate amounts on MBS online.
Please keep in mind that Speech pathologists set their fees, usually higher than the rebate. There's often an out-of-pocket cost for services covered under this program. You pay the full fee and then claim the rebate from Medicare. These expenses count toward the Medicare safety net. Medicare rebates and Private Health Insurance cannot be claimed at the same time.
Please contact Speech Haven for our pricing list. All Medicare rebates will have to be completed by families: https://www.servicesaustralia.gov.au/medicare-claims?context=60092
